Community Access and Integration supports
One aim of integrated care is to enable participants to receive care closer to home, with services focused on keeping them well and avoiding unnecessary hospital care.
Communities are the places where preventive programmes can flourish. We offer community access and integration services which supports people’s self-care and wellbeing, independence and social participation.
Through our SILs (Supporting independence for Life skills) program we
- Foster independence by building on individuals’ abilities and developing community participation, social and daily living skills.
- Increase opportunities for participation in the community with supports that respect personal choices and decision-making, and help people achieve their goals.
- Promote social inclusion and broaden social relationships through the use of community resources and services available to everyone in the community.
- Help young people with a disability who are 18yrs and above who are about to exit school make the transition from school to life as an adult in the community. For example, appropriate transport, budgeting, assistance with housing applications and home setup etc
